Sibling DavidH got the correct answer.

Hook_worms (as parasites) are a literal (and even lethal) pain in the arse (as pars pro toto of the lower intestines).
Hook_worm can also be read as worm on a hook, i.e. bait used by fisherbeings.
They (the worms) are a subgroup of nematodes (literally sewing thread worms) => needle clues
