I think it has more to do with alliteration.
I just looked it up and the saying was already around fully formed in 1510.
The liver was there first, the louse added later. Originally it was something running over someone's liver, and this got modiefied to some small thing and in the end to the louse as something really small and insignificant and also alliterating.

I once saw a man on a subway station accidentally setting his short grey hair on fire. It looked exactly the way iron wool burns, actually quite fascinating. It took some time before the guy even noticed (iirc someone had to tell him) and it took no more than a few pats with the bare hands to extinguish it. If it had not been for the smell, it would have been just funny (I do not think the scalp got burned or otherwise injured).

The hair sits above the person's eye level. When turning the eyes upwards, they disappear inside one's head, therefore preventing site of what lies above. As to the smell, it could come from anywhere.
